In 4 x 2.5 inch red and white box with plastic window orginally for "Esquire Expand-o socks". Box was in a ziploc bag, both box and bag labeled "Bag 18" or "18" in sharpie. Box is labeled in pencil "Surface, Bay Terrace, Qns Co, L.I. N.Y."
